What documents do I need to submit with my claim?

You need to submit scanned copies and/or photos of 4 things, depending on your method of purchase and delivery, as detailed below.

Please note: If your e-bike was shipped to you, do not recycle the box until you take a photo of the shipping label. This is a required document.

In all scenarios below, you will need to submit:

a photo of your new e-bike’s serial number

and a signed copy of your approval email.

Receipt Requirement #1 – Photo of serial number

Receipt Requirement #2 – POS (Point of Sale) receipt

A POS (point-of-sale) receipt from the e-bike retailer. This is usually printed by the cash register and/or the interact/debit machine.

It will look like one of these examples.

Receipt Requirement #3 – Retailer printed receipt

An e-bike retailer issued/printed receipt showing:

  • store name & address
  • logo
  • invoice number
  • date of purchase
  • customer name & address
  • a description of the e-bike purchased
  • the total price before taxes

Note if you include the POS receipt in the scan or photo, please make sure it does not cover any of the required information shown below.

Receipt Requirement #2 – POS (Point of Sale) receipt

A POS (point-of-sale) receipt from the e-bike retailer. This is usually printed by the cash register and/or the interact/debit machine.

Receipt Requirement #3 – A photo of the shipping label that was on the box when you received your e-bike. The address on the shipping label must match the address on your application.

Receipt Requirement #2 – Email purchase confirmation

The email you receive after completing the online purchase and making the payment (typically by credit card).

The following is an example only.

The email must have the following information contained in it:

  1. Date of purchase
  2. Order or invoice number
  3. The email must state that it is a PURCHASE receipt, not an “order.”
  4. Description of the e-bike and price before tax.
  5. Payment method.
  6. Customer’s name, and shipping address.
  7. The shipping address must match the address on your application.

 

Receipt Requirement #3 – A photo of the shipping label that was on/in the box when you received your e-bike. The shipping address must match the address on your application.

Receipt Requirement #4 – Retailer signed approval email

If you purchase your e-bike online for delivery to your home/office/other pick-up location, you must forward a copy of your email approval email to the retailer after your purchase is complete (payment processed). The retailer will forward the email to us with a copy to you confirming the purchase. This email confirmation from the retailer will satisfy the requirement for the signed approval email when you complete the claim process. Please make sure your retailer completes this required step.
