Who can complete this form for an applicant? Someone who is:
- Familiar with the applicant’s financial situation
- A person who works for a relevant organization in a professional capacity such as social work or family services professional, health care professional, First Nation government, Native Friendship Centre, housing provider, religious organization leader, school principle or counsellor.
